Spectinomycin (Trobicin)

Description:

  • Miscellaneous antibiotic

Indications:

  • Gonorrhea due N. gonorrhoeae

Contraindications:

  • None

Precautions:

  • Pregnancy category B

  • Not effective against syphilis

  • Use spectinomycin only in patients intolerant to cephalosporins or quinolones

Adverse Reactions (Side Effects):

  • Single dose reactions: 

    • sore injection site

    • urticaria

    • dizziness

    • nausea

    • chills

    • fever

    • insomnia

  • Multiple-dose reactions: 

    • decreased hemoglobin, hematocrit and creatinine clearance

    • elevations of alkaline phosphatase, BUN and ALT  

Dosage:

  • Administered orally IM injection ONLY deep in upper outer quadrant of gluteal muscle

  • Uncomplicated gonococcal infections: 

    • 2 grams IM

  • Gonococcal infections in pregnancy: 

    • single 2 gram IM dose

  • Disseminated gonococcal infections: 

    • 2 grams q12h

  • Geographic areas having antibiotic resistance: 

    • 4 grams IM total (2 grams per each side)
