    Basic Guidelines for Components of the Gynaecologic History

    1. Patient profile: Name, age, occupation, marital status, gravidity/parity, age of last child
    2. Presenting  complaint
    3. History of presenting complaint (Including direct questioning)
    4. Systematic enquiry
    5. Medical and Surgical History
    6. Drug and Allergy History
    7. Family History
    8. Social History
    9. Past Obstetric History
    10. Gynaecological history
      • Menstrual history
      • Contraceptive history
      • Sexual history
      • History of previous gynaecological investigations and treatment
    11. Summary of the History (This should highlight all the important risk factors obtained in the history)
      • Identify the patient by: Age, Parity, Occupation
      • Give key points of the history in one or two sentences
